Push the connector out from its stored position and swing it out to its fully extended position. Insert the card into an available USB slot on your computer with contacts up. The computer will recognize the device and open a window showing the contents of the device. In this window you should see an icon called “USB Medical Data”. If you have Adobe Reader already installed on your computer, double-click the “USB Medical Data” icon to start the application. Double-click the icon and follow the prompts to install Adobe Reader if: 1. You don’t have Adobe Reader installed 2. You have Adobe Reader but the version is less than version 6.0 3. You aren’t sure if you have Adobe Reader installed 4. The “USB Medical Data” application fails to load When you see a form pop up with “USB Medical Data” at the top, click in the first field to begin entering your information. You can move between fields by clicking in each field or using the TAB key to move between fields. When you have filled in your information to your satisfaction, click on File in the top Menu Bar, then click on Save to save your information to the device. Should you need to print the information, you can click on the PRINT FORM button at the top of the form. Anytime you need to make changes, insert the device, double-click the icon to open the form, make your changes and save the form. When you are done with the form and have saved your information, click on File in the Menu Bar, then click on Close. Remove the device from the port, and swing it back to its stored position. Place the device in a credit card slot in your wallet or purse with the “USB Medical Data” logo visible and your information is now ready to be viewed by Paramedics or other medical personnel.
